Intake Process
At Sherwood Behavioral Health LLC, we know that reaching out for support is a significant first step. We have designed our intake process to be as transparent, welcoming, and efficient as possible, ensuring you feel confident and supported from the very first interaction.
Step 1: Initial Outreach
Whether you are seeking support for yourself, a family member, or are responding to a referral from an external organization (such as school districts, the Department of Social Services, or legal entities), we make the process simple:
Contact Us: You can reach out via our secure online portal, email, or phone.
Initial Assessment: Our team will ask for a few basic details to ensure we can provide the specialized care you need. We prioritize responding to urgent requests quickly.
Step 2: The Consultation Call
Before your first full session, we schedule a brief consultation. This is your opportunity to:
Ask Questions: Get to know our approach to trauma-informed care and medication management.
Clarify Goals: Share what you are looking to achieve in therapy.
Determine Fit: Ensure that our clinical expertise matches your specific needs, including any requirements for court or DSS documentation.
Step 3: Intake Assessment
During your first clinical session, we move beyond the basics to understand your unique story.
Comprehensive Review: We look at your history—including the impact of Adverse Childhood Experiences (ACEs)—to build a tailored treatment plan.
Collaborative Planning: We believe in shared decision-making. Your treatment plan is developed with you, not just for you. This includes setting goals for therapy, school integration, or legal compliance.
Step 4: Ongoing Care & Integration
Once your intake is complete, your journey begins.
Seamless Coordination: If your care involves school districts or DSS, we provide the necessary clinical coordination and documentation to keep you supported across all systems.
Holistic Support: Whether you need medication management, group therapy, or family counseling, we will continue to adjust your plan based on your feedback and progress.
Legal/Court:
Use this secure form to refer a client for mandated treatment. Please attach the court order or specific requirements.
School/Counselor:
For use by school staff. Please ensure parental consent for release of information is on file before submission.
Self/Provider Referral:
For physicians or individuals seeking to begin care.

Our Promise of Privacy and Inclusion
We recognize that the intake process involves sharing sensitive information. We maintain the highest standards of confidentiality. Furthermore, our practice is committed to a racially inclusive and culturally responsive environment where you can feel safe regardless of your background.
